      Online Registration Form

       1. Please fill out the following information to place your child in one of our programs.
       2. After submitting the information below, one of our staffs will contact you for a short interview.
       3. Full tuition must be paid at the Bayside center for your child to be fully registered.
       4. Since our classes are for small groups of maximum of 6-10 students, students must be fully registered to secure their placement in
           our programs.
       5. We accept payments in the form of: Cash, Check (Payable to AGAPE Consulting Group), and Credit Card (5% Processing fee will be 
           added to the tuition)
